Targeted Case Management

 

Targeted Case Management (TCM) services are available to individuals with persistent mental illness, developmental disabilities, cognitive impairments or brain injuries.

TCM services are provided by qualified staff, who have a bachelor’s degree in a human service related field and extensive experience working with these individuals.

 

TCM activities include:

  • Developing an on-going positive relationship with an individual receiving services in order to work most effectively on their behalf

  • Ensuring that the individual has received a comprehensive evaluation and diagnosis

  • Coordinating the development of an individual program plan based on an individual’s needs, goals and desires

  • Advocating on behalf of the individual to assist in achieving their goals and objectives

  • Assisting the individual to obtain appropriate services and living arrangements

  • Coordinating the delivery of services and supports

  • Ongoing involvement and monitoring to ensure continued appropriate provision of services and the appropriateness of the living arrangement

 

Seasons Center provides TCM services for individuals who reside in or have legal settlement in Clay, O’Brien, Dickinson, and Osceola Counties.
