Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is a community based service that provides high-quality, coordinated, and comprehensive services to individuals 17 and older who are experiencing serious mental illness using a multi-disciplinary team approach. Due to the intensity of this customized program, Assertive Community Treatment is only open to people with the most serious needs.

This program is available to residents of Lyon, Osceola, O'Brien, Dickinson, Clay, Emmet, Palo Alto, and Buena Vista counties.

  • Focus on the people who are the most seriously mentally ill that current system is not working for them.
  • Multi-disciplinary team includes nurses, doctors, therapists, substance abuse specialists.
  • Continuous, responsible, accountable, comprehensive care.
  • Team provides treatment at home, in the community

Goals and Key Activities

  • Intensive treatment, support, and rehabilitation.
  • Services typically provided in the home and/or community, and occur as frequently as necessary to assist the client with medication and symptom management.
  • Services can include assistance with activities of daily living, such as grooming and hygiene, shopping and meal preparation, and money management.
  • Other services include assistance with general health management, benefits enrollment and eligibility, transportation/use of public transportation, reducing/eliminating the use of alcohol and drugs, finding/keeping employment, and making/keeping friends.
