Assertive Community Treatment (ACT)
Assertive Community Treatment (ACT) is a community based service that provides high-quality, coordinated, and comprehensive services to individuals 17 and older who are experiencing serious mental illness using a multi-disciplinary team approach. Due to the intensity of this customized program, Assertive Community Treatment is only open to people with the most serious needs.
This program is available to residents of Lyon, Osceola, O'Brien, Dickinson, Clay, Emmet, Palo Alto, Buena Vista, and Sioux counties.
- Focus on the people who are the most seriously mentally ill that current system is not working for them.
- Multi-disciplinary team includes nurses, doctors, therapists, substance abuse specialists.
- Continuous, responsible, accountable, comprehensive care.
- Team provides treatment at home, in the community
