Certified Community Behavioral Health Clinic (CCBHC)

Certified Community Behavioral Health Clinic (CCBHC) is designed to provide a comprehensive range of mental health and substance use disorder services to vulnerable individuals, such as adults with serious mental illness.

The purpose of CCBHC is to improve the quality of community behavioral health services through a comprehensive approach by:
  • expanding access to care
  • stabilizing people in crisis
  • providing the necessary treatment for those with the most serious, complex mental illnesses and substance use disorders
Services include:
  • Care Coordination
  • Psychiatric Rehabilitation
  • Health & Wellness Coaching
  • Nurse Support
  • Peer Recovery Support
  • Targeted Case Management
This system of care emphasizes recovery, wellness, trauma-informed care, and physical-behavioral health integration.

Seasons Center and its affiliates (Autumn Center, Camp Autumn etc.) firmly believe that each individual, family, group, or couple are treated with the utmost respect in their care. Seasons Center and its affiliates believe that recovery is self-directed, individualized, person-centered, helps to empower the client by using a strengths-based and value-based approach that instills hope, respect, and responsibility to their own care and direction in their lives. In regards to overall care, Seasons Center and its affiliates believe in evidenced based practices as well as non-linear and holistic care.  Seasons Center and its affiliates believe and practice strict privacy and confidentiality practices as well as give honor to the patient’s journey and advocate on behalf of the patient whenever possible.
