Emergency Services

Emergency Services are provided 24 hours per day 7 days per week to assist individuals and families who have an emergent need. When you dial our FREE 24/7 Crisis Line at 844-345-4569, you will speak to a caring Seasons employee who will help connect you with the services you need.

Seasons provides these services when:

  • Individuals or family members feel that they or a loved one are a danger to themselves or others.
  • Individuals feel overwhelmed and need to speak to a crisis counselor right away.
  • On-site assistance arising from a traumatic event such as a death or injury in the workplace, natural disaster, car accident, suicide, homicide, or other such traumatic events.
988 Suicide & Crisis Lifeline:
The 988 Lifeline provides 24/7, free and confidential support for people in distress, prevention and crisis resources for you or your loved ones. 

Mobile Crisis Services

24/7 Crisis Phone Line
By calling 844.345.4569 callers speak with a professional who will provide support, screen for safety, stabilize the crisis, and provide appropriate referrals.

Mobile Crisis Services
Mobile Crisis services are available in Buena Vista, Clay, Dickinson, Emmet, Lyon, O’Brien, Osceola, Palo Alto, Plymouth, and Sioux counties. The Mobile Crisis Team will be deployed when necessary to provide a face-to-face crisis screening/intervention. Anyone experiencing a mental health crisis may call the 24/7 crisis line. Mobile crisis can be requested if the individual needs in-person support and/or connected to other resources. We partner with law enforcement, hospitals, schools, medical clinics, courts, and HHS to respond quickly and safely to the location. 

Care Coordination
The Mobile Crisis team provides ongoing stabilization services through care coordination for up to 30 days for individuals that have accessed mobile crisis services. The team assists the individual in obtaining outpatient mental health care with their own provider or be linked to a community resource for further assessment and treatment.

Crisis Stabilization Community Based Services
Seasons is an accredited Crisis Stabilization Community Based Services (CSCBS) agency offering services designed to de-escalate a crisis situation and stabilize an individual following a mental health crisis.  The goal of this service is to keep an individual in his/her home/community, somewhere that is less intensive and restrictive than an inpatient hospital.  Services are provided in-person or virtually and can take place where the individual lives, works or recreates. CSCBS services are available for adults and children in in Buena Vista, Clay, Dickinson, Emmet, Kossuth, Lyon, O’Brien, Osceola, Palo Alto, Plymouth, Sioux, Winnebago and Worth counties.  If eligible, CSCBS care will include the following for up to five days:

  • In person, hour-long appointments at a location of your choosing to work on skill building
  • Daily virtual appointments with a mental health professional
  • Development of a written, short-term stabilization plan
  • Connection to medication, counseling, and peer support services as well as linkage to ongoing services within the community

Insurance information will be collected during the intake. If you have insurance, your insurance company may be billed for some of the services you receive. If you have questions about this, or how to access financial support, please speak with the Seasons staff assisting you with this document.

[Alex's Place] Crisis Stabilization Center

Alex's Place Crisis Stabilization Center

Provides services to adults 18+ experiencing a mental health crisis which does not require an inpatient level of care. The crisis center is appropriate for individuals that do not require hospitalization but need symptom stabilization assistance and skill-building services to regainstability and wellbeing.
  • 4 bed facility offering short term care for up to 5 days
  • Residential care setting with 24/7 supervision
  • Provides therapy with a licensed mental health therapist and behavioral health skill building with a crisis team member each day
Examples of a mental health crisis include but are not limited to:
  • suicidal ideation
  • behavior that could pose risk to themselves or others
  • anxiety
  • depression
  • substance abuse in combination with increasing mental health symptoms

Alex Jacobsen

Alex’s Place is named in honor and memory of Alex Jacobsen. Alex was a motorhead. He had his CDL and loved driving the semi during farming season. One of his favorite pastimes was being on Lake Okoboji. Alex was both a quiet introvert and an outspoken jokester that made people roll with laughter. He was loyal to his family and always involved with his nieces, nephews and siblings. Alex liked the Minnesota Twins, Iowa Basketball, country music, driving country roads and looking at acreages.

Alex was also no stranger to pain and was troubled for a very long time. From a young boy, he always felt there was something different or off about himself. He was incredibly shy and lacked self esteem. His adult life was plagued with mental hardship which included many diagnoses that affected his relationships and his ability to do what others often found easy. No stone went unturned in his family’s attempts to save him from himself.

If we can learn anything from Alex, it’s that mental illness is real and it doesn’t discriminate.
